Directory Information Notice and Opt-Out Form (For Parents/Guardians)

Parents/Guardians may out of sharing some of their child’s directory information when schools are celebrating students’ achievements or noting their participation in activities with the entire school community. 

School-based publications and announcements of honors, awards or other recognition or participation in school activities, including:

  • Graduation and honor rolls
  • Receipt of scholarships or awards
  • School publications that likely include student names and photographs, such as yearbooks, playbills, graduation lists, and participation in school sports and other activities
