Directory Information Notice and Opt-Out Form (For Parents/Guardians)
Parents/Guardians may out of sharing some of their child’s directory information when schools are celebrating students’ achievements or noting their participation in activities with the entire school community.
School-based publications and announcements of honors, awards or other recognition or participation in school activities, including:
- Graduation and honor rolls
- Receipt of scholarships or awards
- School publications that likely include student names and photographs, such as yearbooks, playbills, graduation lists, and participation in school sports and other activities
If you choose to OPT OUT of directory sharing please complete the below letter and return Attention: Judy Rea, Parent Coordinator Room 116
